Job Summary

The Intake Specialist (IS) is responsible for coordination and scheduling timely access to Hospice services. IS provides back up administrative support to Admission Nurses and possesses a thorough understanding of Medicare, MediCal, Private Insurance, and underfunded reimbursement. The IS is expected to have experience and an understanding of the Conditions of Participation of the Medicare Hospice Benefit and associated regulations as well as end of life care services.

Essential Responsibilities

Intake/Referrals

  • Receives referrals/requests for information from referral sources and community members.
  • Provides accurate information regarding hospice services & funding in a professional and sensitive manner.
  • Utilizes established screening process demonstrating compliance with HSCC admission criteria.
  • Alerts Sr. Director of Operations, Director of Nursing or designee of any issues that require immediate action
  • Obtains and sends all medical records to Medical Director(s) and Admission Nurse(s) to assist in their certification of a prognosis of < 6 months
  • Coordinates discharge, delivery of equipment, supplies required to initiate hospice care in environment of patient choice.
  • Documents all conversations and actions taken within EMR
  • Coordinates and collaborates with admission RN on scheduling, assignments and admission process per agency protocols
  • Documents any referrals that have been determined not to meet hospice medical eligibility criteria by Hospice Medical Director.
  • Supports department efforts in achieving agency goals

Secondary Responsibilities

Secondary responsibilities include, but are not limited to, the following and other duties as assigned.

Administrative Support

  • Enters prospects and new patients into EMR.
  • Verifies benefit status and follows up on insurance authorization as needed.

Systems

  • Participates in the development of system changes and enhancements.
  • Participates in special projects as needed

Diversity Access

  • Provides information and facilitation to assist all patients and families with English as a second language in making informed decisions regarding hospice care, by utilizing the Language Line when necessary

Compliance

  • Ensures all HIPAA Privacy and Security Regulations, Medicare Conditions of Participation, and all other regulatory/compliance requirements are understood and followed.

Requirements

Education/Experience

  • Bachelor's degree preferred
  • 1-3 years of administrative experience
  • Hospice experience preferred

Skills Required

  • Advanced computer literacy including word processing, spreadsheet and electronic medical records data entry.
  • The ability to work independently
  • Strong verbal and written communication skills
  • Excellent internal and external customer service skills

Certifications, Licenses, Registrations

  • Pass OIG/GSA review and all required background checks

Physical Demands

  • Strength to lift and carry materials weighing up to 10 pounds
  • Vision to read printed materials and a computer screen
  • Hearing and speech to communicate in person and over the telephone

Work Environment/Environmental Condition

  • Must possess mobility to work in a standard office setting and to use standard office equipment, including a computer
